Reid Health Extends Continual Care to Patients in Their Homes through Vivify Health Pathways

PLANO, Texas, March 14, 2017 /PRNewswire/ -- In its inaugural use of remote patient monitoring, Reid Health is winning overwhelming patient adoption. The health system, which serves patients in west central Ohio and eastern Indiana, credits a 90 percent patient compliance rate to attentive onboarding by care coordinators and patient-friendly technology from Vivify Health, the pioneer and market leader of remote care via mobile devices.

Known as the health system "with a personal touch," Reid Health has intentionally designed its continual care program to foster engagement and higher satisfaction among patients dealing with isolating chronic conditions that leave them vulnerable to gaps in care between clinical visits.  The program continuously connects patients to their care teams and also equips them with self-reporting tools, on-demand education and more, all within the Vivify Pathways™ solution.

Families and patients give Reid's remote monitoring high marks

With the continual care program launched just this past November, Reid Health is currently collecting benchmark data on reduced hospital readmission rates and increased medication adherence. Already evident is the patient compliance rate of 90 percent, plus significantly positive feedback from patients and their family members.

A smooth transition of patients into the continual care program is a vital beginning to high engagement and compliance. To that end, each patient in the continual care program receives a remote monitoring kit from Vivify Health that includes a tablet with animated videos that walk patients through an easy set-up, and various wireless health devices to collect the patient's biometric data.

At any time patients can request a video chat, look up information about their condition, review their care plan instructions, and at scheduled intervals, share biometric data with care team members who monitor for any anomalies in the patient's condition. Such events can be quickly acted on, often with a mild intervention in the patient's own home that heads off a more acute deterioration which could end with a hospital admission.

To rein in costs while increasing patient engagement, more health systems are turning to Vivify's Pathways remote monitoring via any mobile device.
