HRS Selects Lacuna Health to Re-Launch CareConnect™ Clinical Monitoring Service

Health Recovery Solutions (HRS), a leading nationwide remote monitoring company, has partnered with Lacuna Health, a leading nationwide registered nurse-led clinical engagement company, in its re-launch of HRS CareConnect™, a service offering continuous monitoring and extended coverage to care providers. HRS selected Lacuna Health for its ability to offer telehealth and Remote Patient Monitoring (RPM) via its 24/7 RN-led remote clinical support and patient engagement model.

The Lacuna-powered clinical monitoring service will augment HRS’s existing capabilities and accelerate telehealth program growth of the legacy HRS CareConnect program. For Lacuna Health, this partnership further solidifies efforts to become a leading national provider of Centers for Medicare & Medicaid Services compliant chronic care management (CCM) and RPM programs.

HRS CareConnect™ offers a range of care management services to support the HRS telehealth platform. Clients who use HRS CareConnect™ will have access to Lacuna Health’s RN-led contact center team, who provide care coordination, health assessments, and remote monitoring to give patients the tools and support they need to remain healthy at home.
